To apply for an 804 visa, prepare the following set of documents:
– The passport on the basis of which you entered Australia.
– Birth certificate – you must specify the names of both your parents.
– Birth certificates of all family members included in the application. If someone cannot provide a birth certificate: a family registration document, an identity document (ID) issued by a government agency, a court-issued document certifying your identity.
– Four recent photos of each applicant (on a white background, size 45×35 mm).
